Mini Grants are offered once per year. Apply for up to $500 to support health and well-being in your department!
All Wellness Ambassadors who want to apply for a mini-grant must first:
- Complete the Healthy Department Certification. If you have been granted "striving for certified healthy," "certified healthy," or "gold certified healthy" in the last three years, you will not need to complete this certification process again.
- Conduct a Needs Assessment within the last year. Reference and use this sample assessment to create your own via Survey Monkey, Qualtrics, Smartsheet, or another tool that works for your department.
- Speak with department Account Manager/Business Office to confirm that the outlined purchasing process is feasible. If awarded a grant, this person will be included on all mini grant communication.
Grant applications are currently closed. Funds awarded must be spent by Friday, May 21, 2027.

Once mini grant activities have been completed, please submit an Outcome Report (Smartsheet form) detailing how your grant activities went.
